All adults regardless of health or age can complete an Advanced Directive. Today you can speak and make decisions, but what if you had a sudden accident or illness and were unable to speak for yourself? Who would you trust to make medical decision for you? What do you what them to know?

An Advanced Directive is a legal document that stares your medical care wishes and lists the names of who you appoint to make those decisions, honoring your choices if you were unable to do so yourself. With this free conversation, we can help you document your healthcare choices.

This service is provided by the Bucks County Health Improvement Partnership. To register, please call 267-291-7882. You must register for an appointment time slot. It is approximately one hour.
